Skilled Nursing Services

Assisted living is for seniors who don’t need around-the-clock nursing care but who could use help with activities of daily living (ADL). ADLs include assistance with eating, medication reminders, bathing, dressing and getting out of bed.

Skilled nursing care refers to a patient’s need for care or treatment that can only be performed by licensed nurses. This type of care is usually offered in hospitals, assisted living communities, Life Plan Communities, nursing homes and other certified locations. Skilled nursing is mostly regulated by the U.S. Department of Health and Centers for Medicare & Medicaid Services (CMS)

Our client-centered approach goes beyond case management to offer integrated life and care coordination services. Our highly skilled team of professionals can help manage the medical and personal aspects of life. Our medical advocacy team consists of highly skilled Registered Nurse Patient Advocates with experience working in diverse settings. Our RN Advocates have years of varied clinical experience and are assigned to our clients based on the client’s needs and their expertise. Our team members work collaboratively using their skills and experience to support our clients in all aspects of their life.

Physical, Occupational, & Speech Therapy

Physical Therapy: After an illness, surgery or hospitalization, a rehab care team will create a personalized plan for the most optimal outcome.

Occupational Therapy: Help adapting to social and physical environments through specialized therapies for getting dressed, memory training and coordination exercises.

Speech Therapy: Addresses communication issues and swallowing dysfunction. Speech and language pathologists design a treatment plan to help with language ability, provide alternate communication strategies and give appropriate diet recommendations.

Medical Social Worker

A medical social worker, sometimes referred to as a health care social worker, helps patients and their families cope with illnesses. They help patients understand their illness or condition and provide them with information about the resources available to them to cope with the emotional, financial, and social needs that arise with a diagnoses. As an advocate for the patient, a social worker assures that patients’ emotional and social needs are met through their medical treatment. Medical social workers work with patients of all ages, from infants to the elderly as well as their families.
